Screening Form: Policies & Information
| NEW CLIENT RESERVATION & SCREENING FORMS Screening form policies and information. All potential clients must first fill out my a screening form. You must fill out this form completely and accurately before I will even look at it. If you did not fill out the form completely or correctly it will be deleted. If it's deleted you will need to fill the form out again before you can be approved and able to set an appointment. If I accept you as a client you are given a Client ID and you can use that to request and reservation using the online form. Filling out the screening form does not guarantee you will be accepted as a client. If I choose not to meet you for any reason, your form and all information will be deleted immediately. I'm happy to tell you why I denied your request but I'm not likely to change my mind. This information is solely for my own personal security; all information is confidential and secure. I understand there is a heightened concern for discretion and anonymity and I assure you this is my biggest concern as well. However, I also need to establish the legitimacy of the individuals I meet. I use this information to make sure that you are, at the very least, a reputable person. This is for my comfort and safety. If I do not feel safe or comfortable with you we are not going to have much fun. I’m a very busy person, I can not stress this enough. It may take me several days before I can get to my computer and several more to confirm the information on the reservation form/screening form so it’s best to book your first appointment a week or more ahead of time. Please be aware that I turn down most of the people that fill out this form. I am very picky and I only book with people I think I will "click" with. I strongly suggest reading the entire FAQ page before filling this form out. |

Provider References You will be asked to provide two (2) references from current and reputable providers. I do not accept references from managers, bookers or services as I wish to speak directly to the provider about your demeanor. | Employment Verification This is option is for people without qualifying references. You can also use employment verification. I will attempt to verify the authenticity of the company, your employment. I will never divulge the pretense of my inquiry. |
Bowser not working with the webforms?
Some browsers do not allow them to use the webform so I have an optional "Copy, Fill and Send" form. The optional "Copy, Fill and Send" version of the Screening Form is a little more work but not complicated at all . It's actually quite easy, youcopy all the text, fill it out and email it to me at: lavishfixation@gmail.com
